Communication Request Form

- In order to accommodate existing projects, please allow for a minimum of 15 working days once the request is submitted.
- If your request includes contacting a targeted segment of alumni and friends, a separate request form may be required by contacting UCO Advancement Services.
- If applicable, the UCO Office for Advancement team will assign an appeal code to facilitate data entry upon receipt of funds and recurring gifts. The appeal code should be incorporated into the design of all return cards and forms in an inconspicuous but clearly visible location, preferably the lower right-hand corner of the front of the print piece.
- The UCO Advancement communication team will review any suggested changes with you as necessary. The UCO Advancement communication team must have the opportunity to review the final version of any promotional pieces associated with the communication request.
